Services:   Please note....our office is open to walk-in traffic 9AM to 11:45, 1:15PM to 3:15PM.

Most services can be done by mail with most forms available on this website.  Call 812-275-3234, ext. 2610 or 2612 with questions or to have forms emailed directly to you.   Some birth records cannot be issued while you wait depending on availability on the state's site.

Birth and death records are available beginning with the year 1882. There is a limit of 5 certificates per name.  We do not accept personal checks, debit or credit.  Money orders or cashier's checks for mail-in requests, cash in office.

IMPORTANT: If you want to obtain any records in person, via office walk-in, you must arrive by 3:15 p.m.  Birth certificates may not be issued same day, as each time a record is issued it must be verified for accuracy with the Indiana State Department of Health.  If there is missing information or information that does not match the application you fill out, an email is sent to the state for verification and is answered at the state's convenience.

PLEASE NOTE:  Marriage Licenses can be obtained from the Clerk's office in the Lawrence County Courthouse. For more information call 812-275-7543.  We do not hold marriage licenses or divorce decrees.

 

Identity theft is a growing concern in the U.S. A birth certificate is a prime piece of documentation used to establish false identities. For your protection, the Vital Records Division of the Lawrence County  Health Department and the Indiana State Department of Health make the following recommendations:

  • Do not keep your birth certificate on your person unless needed for a specific purpose.
  • Do not carry your Social Security Card on your person unless needed for a specific purpose.
  • Remove your Social Security Number from your driver's license, checks, etc.
  • If your wallet or purse is stolen with these pieces of identification, check your credit ratings at least every 6 months.
  • Keep these pieces of I.D. (along with all legal documents) stored in a safe place when not in use.

Birth/Death Certificate Request Forms

Identification is REQUIRED when obtaining a birth or death record. To request a certified copy of a VITAL record you MUST be 18 years of age or older.

Obtaining Vital Records

By mail: Complete the necessary application and mail it with the proper fee*, ID and a self-addressed stamped envelope to:

WE DO NOT ACCEPT PERSONAL CHECKS!  (Money order or cashier check).

WE DO NOT ACCEPT BIRTH OR DEATH CERTIFICATE REQUESTS VIA EMAIL, FAX, OR PHONE!

Acceptable Forms of Identification, only requires on of the following   (**All documents must be current and valid**)

  • Government issued Driver's License/State ID (must be current).
  • A valid, current passport
  • U.S. Military I.D.
  • Veterans I.D.
  • Mexico Consular I.D. (Green)

Identification MUST contain your signature. If you are making a request via mail, please photocopy your identification front and back; do NOT send the original document!

Amending Vital Records

The following options are available to amend a vital record, if ALL necessary criteria are met:

  • Court Order Change of Name
  • Court Order Determining Parentage
  • Correction Affidavit
  • Paternity Affidavit
  • Paternity Affidavit Upon Marriage

Please do not make any assumptions based on this list. Call the Lawrence County Health Department for detailed information regarding ANY of these procedures.

  • Death Certificates
  • $15.00 per copy
  • Genealogy Search  (Record being searched must be at least 75 years old to be considered genealogy)
  • $5 per name
  • Genealogy Information Sheet (Record must be at least 75 years old to be considered genealogy)
  • $5 each
  • Correction Affidavit
  • $20 (fee does not include a Birth Certificate)
  • Paternity Affidavit Upon Marriage
  • $25 (fee does not include a Birth Certificate)
  • Paternity Affidavit
  • $25 (fee does not include a Birth Certificate)

In order to have a record pulled, you must arrive at our office by 3:15 pm.
