Lawrence County Immunization Information
Childhood immunizations are given at the Lawrence County Health Department BY APPOINTMENT, Monday & Tuesday. Appointments are scheduled 9:00 to 11:45 a.m. and 1:30 to 3:45 p.m. The Lawrence County Health Department is a member of the Children & Hoosiers Immunization Registry Program - C.H.I.R.P.
IMMUNIZATION RECORDS ARE REQUIRED for all patients.
Copies of your immunization record may be obtained at the Lawrence County Health Department for a fee of $2.00. You will need to fill out an Immunization Record Request Form & an I.D. is required. For a walk-in request, you must arrive at our office by 3:45 p.m. No records are issued after 4 p.m.
Immunizations are offered for Infants, Children entering Kindergarten, and Students 11 thru 18 Yrs. of age needing the State required vaccines. There is an $8.00 Administration Fee per shot. Tdap, Varicella #2 & Menactra for School Entry. Medicaid patients are required to provide their Medicaid Card. Immunization Records are Required for all patients.
Adult Vaccines are also offered by appointment for our patients 19 years of age & up. The vaccines available are Tdap, Menactra, MMR, Hep A, Hep B, Hep A/B, Flu & Pneumonia. These are offered from our Private Pay Vaccines. The fee for these vaccines are presently out of pocket, since we currently are unable to bill insurance. (See Services & Fees).
For information regarding vaccinations for travel to foreign countries, please visit the Centers for Disease Control web site. Some travel vaccines may be available at the Monroe County or Washington County Health Departments.
HPV Vaccines are available:
- Approved for boys and girls, ages 9-26
- Most effective if given between ages 11 and 12
- Protects against HPV strains that cause genital warts, cervical, penile and anal cancers

Copies of your Immunization Record may be obtained @ the Lawrence County Health Dept. for a fee of $2.00. You will need to fill out an Authorization to Release Request Form & I.D. is required.

Adult Vaccine Fees:
- Hepatitis A (adult)
- $40 per dose
- Hepatitis B (adult)
- $40 per dose
- Hepatitis A&B combo (adult)
- $55 per dose
- Tetanus & diphtheria (Td & Tdap) (adult age 19 & up)* A script from the physician is required with injury
- $40.00 per dose
