Childhood immunizations are given at the Lawrence County Health Department BY APPOINTMENT.  Appointments are scheduled 9:00 to 11:30 a.m. and 1:30 to 3:15 p.m. The Lawrence County Health Department is a member of the Children & Hoosiers Immunization Registry Program - C.H.I.R.P. 

IMMUNIZATION RECORDS ARE REQUIRED for all patients.

Copies of your immunization record may be obtained at the Lawrence County Health Department for a fee of $2.00. You will need to fill out an Immunization Record Request Form & an I.D. is required. For a walk-in immunization record request, you must arrive at our office by 3:30 p.m. No records are issued after 4 p.m.

Immunizations are offered for Infants, children entering kindergarten, and students 11 thru 18 Yrs. of age needing the State required vaccines.  There is an $15.00 administration fee per shot.  Immunization records are required for all patients.  Insurance, medicaid cards and/or payment are required at time of service.

Adult Vaccines are also offered by appointment for our patients 19 years of age & up.  The vaccines available are offered from our Private Pay Vaccines.  Insurance, medicaid cards and/or payment is due at the time of the visit.  Vaccines available are Flu, High Dose Flu, FluBlok, Hepatitis A, Hepatitis A/B, Hepatitis B, HPV 9, MCV 4, MMR, Pneumonia, Polio, PCV 20, Meningitis B, Shingrix, TD, Tdap, and Varicella.  There is an $15.00 administration fee per shot.

For information regarding vaccinations for travel to foreign countries, please visit the Centers for Disease Control web site.  Some travel vaccines may be available at the IU Health Travel Center or Washington County Health Department.

HPV Vaccines are available:

  • Approved for boys and girls, ages 9-26
  • Most effective if given between ages 9 and 12
  • Protects against HPV strains that cause genital warts, cervical, penile and anal cancers
  • "About 79 million Americans are infected with HPV." - CDC, 201
